2024 OCC Day Camp Waiver and Release Form
Release Form
Please complete this form and all of the required fields for each child attending OCC Day Camp. This form should only be completed once per summer. If there are any questions, please email Teresa Christoff, OCC Family Pastor, at teresa@ofallonchristianchurch.org.

Technology Release - I understand that the usage of technology, including computers, internet, and cell phones, is directed as necessary by the camp instructors. Responsible and appropriate use of technology is required and expected by all campers. If camp staff becomes aware that my child is involved in any unacceptable situations, the parent/guardian will be contacted. Depending on the severity of the incident, my child may be suspended or dismissed without refund. Cell phones are to be placed on silent and put away at all times throughout the camp day, unless as directed as necessary by the camp instructors. Inappropriate use of cell phone will warrant a phone call to parent/guardian and further action may be taken. OCC is not responsible for any loss or damage to personal technology.
Medical Information - EPI-Pen - If your camper requires an EPI-PEN, you will be required to bring the EPI-Pen to camp every day and it will go home after camp each day. It will be held by the camp assistants throughout the camp day. If the EPI-Pen is forgotten upon arrival, the camper will not be able to attend.
Medical Information - Inhaler - If your camper requires an inhaler, you will be required to bring the inhaler to camp every day and it will go home after camp each day. It will be held by the camp assistants throughout the camp day. If the inhaler is forgotten upon arrival, the camper will not be able to attend.
Hold Harmless Agreement -
I understand that I cannot drop my camper off earlier that 10 minutes prior to camp start time, and must be aware that camp staff is present when I drop them off. I understand an authorized individual must pick up the camper promptly at the end of camp. Supervision is not provided outside of the camp day.
I acknowledge that the activity listed herein may result in injuries to the participants. I further acknowledge and hereby assume the risk of any injuries resulting from the activity listed herein. I will not hold OCC, or its employees, administrators, board members, volunteers, and partner organizations responsible in the event of an accident, injury, or loss.
I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures may be performed or prescribed by the attending physician and/or paramedics for my child and waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of emergency.
Disciplinary Action and Agreement -
I understand OCC may suspend or dismiss without refund my child from the program for any reason that is deemed harmful or disruptive to the other participants or to OCC staff. I understand campers are expected to follow instruction from camp staff which includes refraining from by not limited to hitting, threatening, harassing, bullying, or impeding the enjoyment of camp by other participants. Not all behaviors will result in repeated warnings; serious offenses may result in immediate removal without refund.
